Esomeprazole 40mg (GR) & Domperidone30mg (PR) Capsules
Introduction
This combination medication contains two active ingredients:
- Esomeprazole 40 mg: A proton pump inhibitor (PPI) that reduces gastric acid production by blocking the final step of acid secretion in the stomach.
- Domperidone 30 mg SR (Sustained Release): A dopamine receptor antagonist that enhances gastrointestinal motility and controls nausea and vomiting. The sustained release (SR) formulation ensures a gradual release of Domperidone for a prolonged therapeutic effect.
Uses
- Treatment of acid reflux, gastroesophageal reflux disease (GERD), ulcers, and other conditions caused by excessive stomach acid.
- Management of nausea, vomiting, and delayed gastric emptying associated with Parkinson’s disease treatments.
Administration
- Take at least 1 hour before meals.
- Swallow the capsule whole without crushing or chewing.
- If swallowing is difficult, the capsule may be opened, and its contents sprinkled on one tablespoon of applesauce; swallow immediately without chewing the granules.
Storage
- Store at room temperature in a tightly closed container.
- Protect from heat, moisture, and direct light.
Mechanism of Action
- Esomeprazole inhibits the H+/K+ ATPase enzyme system in gastric parietal cells, effectively reducing gastric acid production.
- Domperidone acts as a dopamine receptor antagonist outside the blood-brain barrier, improving gastric motility and reducing nausea by enhancing esophageal sphincter pressure and promoting faster gastric emptying.

Contraindications

- Not recommended during breastfeeding due to potential harmful effects on infants.
Safety Advice and Warnings
- Allergic Reactions: Seek immediate medical attention if symptoms such as itching, swelling (hands, face, mouth), trouble breathing or swallowing occur.
- Vitamin B12 Deficiency: Long-term use may reduce vitamin B12 absorption. Inform your doctor if you have related concerns.
- Serious Gastric Conditions: Monitor for severe gastrointestinal symptoms, especially if used alongside antibiotics.
- Hypomagnesemia: Long-term use or concurrent use with digoxin or diuretics may lead to low magnesium levels; regular monitoring may be necessary.
